All Pennwood Cyber Charter School students must provide evidence of their immunizations, or provide a Statement of Immunization Exemption, as required by the state of Pennsylvania, to be eligible for enrollment. Acceptable Proof of Immunization (all types require the student's name, date of birth, and vaccine dose and date information):
- Immunization booklet
- Official school record or written statement from the former school district
- Healthcare provider or Health Department printout
- Pennwood Cyber Charter School Immunization Record/Statement of Exemption
These forms must be signed by the parent or legal guardian and, if applicable, by a health care provider.
